This report describes a mixed-dentition patient with bilateral malposition mandibular lateral incisors who was treated in two phases without extractions. Early treatment of a mandibular canine-lateral transposition increases the chance of success with a nonextraction approach. In our patient’s first phase of treatment, up righting and repositioning of both mandibular permanent lateral incisors cleared the paths of eruption for the first premolars. The second phase focused on regaining space for the slow-erupting mandibular canines while correcting the dental discrepancies in both arches. The treatment plan allowed a suitable nonextraction solution for this patient.
