Hallazgos en la evaluación cardiológica en pacientes pre-quirúrgicos del Hospital Infantil Dr. Robert Reid Cabral en el periodo noviembre 2017 - abril 2018

La evaluación preoperatoria en la práctica médica es frecuente, tanto en niños como en adultos.Algunos investigadores han estudiado si la intervención del especialista cardiólogo en la evaluación de dichos pacientes previo a realización del procedimiento quirúrgico es beneficiosa. Éstos han demostrado, que la evaluación permite identificar condiciones médicas, que deben ser conocidas por el especialista, cirujano pediatra y demás subespecialidades y los riesgos y beneficios que pudiesen ocurrir durante el procedimiento.
Se llevó a cabo un estudio observacional, descriptivo y transversal, de tipo prospectivo, en el cual utilizamos como instrumento de recolección de datos un cuestionario, dirigido a pacientes que serían intervenidos quirúrgicamente. Se escogieron 100 pacientes que asistieron a la consulta de Cardiología Pediátrica para realizarse una evaluación pre-quirúrgica.
El 75 por ciento de los pacientes fueron masculinos.Un 25 por ciento se encontraron en el rango de edad de 5-7 años.El 78 por ciento de los pacientes provinieron de la zona urbana.El departamento de CirugíaGeneral fue el servicio que más pacientes refirió para evaluación cardiológica pre-quirúrgica con un 30 por ciento, seguido por Otorrinolaringología, con un 26 por ciento.Los motivos de evaluación más frecuentes fueron por amigdalitis recurrente con un 15 por ciento, hipospadia con un 12 por ciento, fimosis y criptorquidia con un 8 por ciento. El 44por ciento de los pacientes no contaban con antecedentes patológicos y el 56 por ciento, sí presentaron historia de algún padecimiento.En el examen físico realizado en la consulta, el 96 por ciento, no presentó alteración cardiológica, en cambio el 4 por ciento, presentaron soplos sistólicos I-II/VI en BPI alto. El 95 por ciento de los EKG realizados se encontraron dentro de los parámetros normales.El 95 por ciento de los ecocardiogramas se encontraron dentro de los parámetros normales y el 5 por ciento presentó patologías sin contraindicación quirúrgica.En los hallazgos radiográficos encontrados, el 83 por ciento de los pacientes reportó una silueta cardíaca y flujo pulmonar normal, el 14 por ciento presentó cardiomegalia radiográfica con flujo pulmonar normal, sin patología cardiaca importante al evaluar el reporte del Eco 2D.
The preoperative evaluation in medical practice is frequent, both in children and adults. Some researchers have studied in the intervention of the specialist in cardiology in the evaluation of the patient's symptoms and in the performance of a surgical procedure. These have shown that the evaluation allows the medical conditions to be identified, which must be recognized by the specialist, the pediatric surgeon and others and the risks that arise during the procedure.

The most recent evaluation reasons were recurrent tonsillitis with 15 percent, hypospadias with 12 percent, phimosis and cryptorchidism with 8 percent. 44 percent of the patients did not have a medical history and 56 percent had a history of some disease. In the physical examination carried out in the consultation, 96 percent did not present cardiological alterations; whereas 4 percent presented systolic murmurs I-II / VI in high BPI. 95 percent of the ECGs were performed within normal parameters. 95 percent of the echocardiograms found in normal parameters and 5 percent presented pathologies without surgical contraindication. In the radiographic findings found, 83 percent of the patients reported a cardiac figure and normal pulmonary flow, 14 percent with radiographic cardiomegaly with normal pulmonary flow, without significant cardiac pathology when evaluating the 2D Echo report.
