El dolor torácico agudo es uno de los motivos más frecuentes de consulta en
servicios de urgencia. Una de sus causas más relevantes son los síndromes
coronarios agudos. Sin embargo, existen muchas otras etiologías de dolor torácico
agudo dentro de los diagnósticos diferenciales, algunas de ellas igualmente graves.
En esta revisión se presentan las características clínicas que ayudan a diferenciar
estos diversos cuadros, como también sus características electrocardiográficas y
radiológicas. Se hace mención a los biomarcadores de daño miocárdico, de especial
importancia dada su alta sensibilidad y especificidad. En las urgencias hospitalarias al
paciente que consulta por dolor torácico, en primer lugar se le deben tomar las
constantes vitales, hacerle un electrocardiograma y radiografía de torax; se le hará una
evaluación inicial urgente mediante la clínica, la lectura del electrocardiograma y
radiografía, luego de la evaluación inicial y que se tenga un diagnóstico presuntivo o
definitivo se procederá al tratamiento que dependerá de si la etiología del dolor es no
cardiogénico o cardiogénica que puede poner en riesgo la vida del paciente o no. A
partir del diagnóstico el manejo del paciente puede ser ingreso a unidad de cuidados
intensivos, referido a cirugía o tratado ambulatoriamente y ser referido a consulta
especializada. Acute chest pain is one of the most frequent reasons for visits to emergency services. One of its most relevant causes is acute coronary syndromes. However, there are many other etiologies of acute chest pain within differential diagnoses, some of them equally severe. This review presents the clinical characteristics that help to differentiate these various tables, as well as their electrocardiographic and radiological characteristics. Mention is made of the biomarkers of myocardial damage, of particular importance given their high sensitivity and specificity. In hospital emergencies, the patient who consults for chest pain should first have his vital signs taken, an electrocardiogram and a thorax X-ray; an initial evaluation will be made through the clinic, the reading of the electrocardiogram and radiography, after the initial evaluation and a definitive diagnosis, shall be treated depending on whether the etiology of the pain is noncardiogenic or cardiogenic and may or may not endanger the life of the patient, Based on the diagnosis, the patient’s management may be admitted to the intensive care unit, referred to surgery or treated on an outpatient basis, and referred to a specialized clinic.
